CTF Staff Complaint Form
This form can be used by any member when they find a staff member’s conduct and/or actions to be inappropriate or it is felt that a staff member is not fit to run CTF. Forms will only be taken seriously if it is felt by the administration that the staff member in question has been truly unfair. Members are requested to be polite in their complaints and not to make uncalled for jibes at staff members. Complaints judged to be unfairly offensive will not be taken seriously. Complaints will be logged and reviewed in the admin forum. Before filling out the form, please make sure you are aware of both the Rules and the Moderators' Code of Conduct. You can fill out this form in as little or as much detail as you wish (though starred points do require some response), however, more comprehensive complaints are more likely to be taken seriously. Once completed, the form should be sent to an administrator either via PM or email (admin@christianteenforums.com). 1) *Your screen name. 2) *Name of the staff member your complaint is about. 3) *Summary of your reasons for complaining, including rule(s) broken. 4) *Evidence to support your complaint. (Reference to incidents; including links, chat logs, PMs etc.) 5) Actions that could be taken to resolve the situation. 6) Any further comments. |
